Hospice North Shore is a specialist hospice and palliative care service which has been providing quality, professional and dedicated care to patients and families for over 30 years. The hospice is committed to a forward thinking and innovative approach to service development and collaboration.
Working within a Narrative and Collaborative therapies framework, the Family Support Team plays an integral role offering counselling, social work, cultural support, chaplaincy, life review and various support groups.
About the Role
The key focus of the Asian Liaison Social Work role will be to work within a multidisciplinary team improving and enhancing care and support of palliative care patients and their families. The role will respond to the cultural and practical social needs of patients and advise them, and their family/whanau, on assistance and support available from statutory and community organisations. You will also assist individuals and family/whanau to work on Advanced Care Planning (ACP) and to maintain autonomy and dignity in adjusting their care and support needs.
About You
You will have demonstrated experience in social work and experience (or a genuine interest) in palliative care and an understanding of the holistic needs of patients and their families.
Strong oral and written communication will be essential, as will the ability to work collaboratively and effectively in a multidisciplinary team environment and within the community.
