Referrals

Patients are usually referred to Hospice North Shore through a healthcare professional, such as a GP or medical specialist. To refer yourself or a family member, please click here or call us on (09) 486 1688.

Q. | What happens once my referral has been accepted?
Once a referral is accepted, a Hospice Nurse will contact you within 2-3 working days, or, in an urgent case, within a matter of hours. A plan of care will be agreed with you and your family/carers and it will be reviewed regularly.
 
You can also access other Hospice North Shore services such as counselling, social work, physiotherapy and spiritual care through our team.
