Any patient over the age of 18 with an active and advanced life-limiting disease such as cancer, heart failure, motor-neurone disease or multiple sclerosis (MS) may be referred for Hospice support at some point in their illness.
Patients are often referred at a time when they are close to the end of active treatment, or simply when they have particularly difficult symptoms.
Patients who are nearing the end of their life may also want to discuss their preferences for where they die, and know what additional support the Hospice may be able to offer them.
Q. | What happens once my referral has been accepted?
Once a referral is accepted, a Hospice Nurse will contact you within 2-3 working days, or, in an urgent case, within a matter of hours. A plan of care will be agreed with you and your family/carers and it will be reviewed regularly.
